The Science of Exosomes

Exosomes are nanoscale extracellular vesicles, 30–150 nanometers in size, secreted by cells to facilitate direct cell-to-cell communication. They carry bioactive cargo including growth factor proteins, mRNA, and miRNA that instruct surrounding cells to modulate inflammation, recruit stem cells, and regenerate tissue.



For decades, the efficacy of mesenchymal stem cell (MSC) therapy was attributed to transplanted cells differentiating into specialized tissue. It is now increasingly understood that the real mechanism of action is the paracrine signaling of the growth factors, cytokines, and exosomes that MSCs secrete. In most transplant therapies, the majority of transferred cells do not survive the procedure — they leave behind cellular debris that extends the inflammatory phase of healing. Exosome therapy eliminates this problem by delivering the bioactive signaling molecules directly, bypassing the cell entirely.

Why Bone Marrow MSCs Are the Gold Standard Source

Cell source matters. Exosomes reflect the nature and protein profile of the cell they originate from. Bone marrow mesenchymal stem cells are the most extensively studied and characterized cell line in regenerative medicine, supported by over 63,000 peer-reviewed articles on PubMed — compared to approximately 1,200 for all perinatal (birth tissue) MSC sources combined.


Research has demonstrated that bone marrow MSC-derived exosomes show superior immunomodulatory performance during the inflammatory phase of healing compared to placental-derived alternatives. BM MSCs also exhibit greater differentiation capacity, and their complementary extracellular vesicles deliver signals leading to greater differentiation of a patient’s own cells into necessary healing tissues.


Additionally, birth tissue sources carry inherent variability: every birth involves a different mother and fetus, making it nearly impossible for manufacturers to guarantee a consistent end product.     A: PBME Topical™ is intended for topical applications only. Each box contains a single-use 5 mL vial. Once thawed, draw the material into a syringe and apply the product directly onto the area of treatment.


  • Q: How are the exosomes processed?

    A: MSCs naturally produce exosomes. Future Medical Products isolates and purifies them from the cells using a proprietary process in a cGMP facility. This process protects the 3D structure and bioactivity of the proteins and extracellular vesicles from enzymatic breakdown, oxidation, and possible damage during freezing.

  • Q: How is PBME Topical™ stored?

    A: PBME Topical™ is supplied frozen. Store below -40°C in an ultra-low temperature freezer (down to -80°C) for the labeled shelf life of 5 years. If stored between -20°C and -40°C, the product has a 6-month shelf life from receipt. Do NOT store in a residential frost-free freezer or in liquid nitrogen.


  • Q: What is the post-thaw stability?

    A: Once thawed, PBME Topical™ remains bioactive for up to 7 days under refrigeration. Room temperature stability is 6 hours after complete thaw. The product takes approximately 15–20 minutes to thaw at room temperature. Do not refreeze.


  • Q: What happens if my order arrives early?

    A: PBME Topical™ is shipped on dry ice. If the product arrives before your scheduled treatment, do not remove it from the cooler. Place the entire Styrofoam package in a refrigerator (not the freezer) to preserve the dry ice longer.


  • Q: What makes PBME Topical™ different from birth tissue exosome products?

Manufacturer Storage


The samples are meticulously stored at a temperature of -80°C at the FMP facility, ensuring their stability and integrity for future use. When it comes to shipping, they are carefully packaged with dry ice to maintain the extremely low temperatures required, so that the samples remain in optimal condition during transit.

Customer Storage


Store the product between temperatures of -20°C and -40°C. . Ensuring these temperature conditions are met will help preserve the product's quality and efficacy throughout the storage period.

Freezer Restrictions


Do NOT store in a residential-grade frost-free freezer. These freezers undergo repeated freeze-thaw cycles that damage the product.

Post-Thaw


Bioactive for up to 7 days refrigerated. 6 hours at room temperature. Thaw time: approximately 15–20 minutes. Do not refreeze.

Early Arrival


If the product arrives before treatment day, place the entire Styrofoam package (unopened) into a refrigerator to preserve the dry ice. Do not place in the freezer.
